For all UAD FKIP students, please fill out the form below to issue administrative documents of your choice. It will take approximately 3-4 days to issue the document after you fill out the online form.

Read the description of each type of document before filling out the application form for online administrative document to avoid mistakes and misunderstandings.

  1. Student Verification Letter
    This document is for obtaining work benefits, extending BPJS membership, and other related purposes for students whose parents are civil servants or pensioners.
  2. Regular Statement Letter
    This document is used for personal purposes, for example: applying for scholarships outside UAD, participating in TBQ, issuing an ATM card and others.
  3. Graduation Certificate (SKL)
    This document is for graduates who want to make a temporary graduation certificate before the official diploma is issued. Graduation certificate functions as a substitute for a diploma.
  4. Research Observation Permit
    This document is for students who want to carry out observations, for example for research observation.
  5. Research Permit
    This document is for students who need to conduct research in the pre-determined place of research.
  6. Course Assignment Observation Permit
    This document is for students who want to carry out observations or surveys to complete course assignments.
  7. Place Use Permit
    This document is specifically for students who need to use laboratories and other places outside the faculty or in other institutions.
  8. Visitor Permit
    This document is specifically for students who will make a visit to laboratories, museums, or other places.
  9. Acknowledgment Letter
    This document is used to express gratitude after a visit.
  10. Expert Application Letter
    This document is for students who want to request for a lecturer/ teacher to become an expert in validating their materials, media or research questionnaires.
